How to use the Online Report forms

General information: 
To fill out the form, click in the box you wish to enter data in and just type it in.  If the question asks for you to choose something (the question will have numerous choices beneath it, each with a small circle next to it), then click on the circle next to the best answer for the question.  Unless told otherwise, you may only choose one answer to this question.   

When you have completely finished filling out the form and no questions are left blank, you can click on the button which is labeled "submit" and send it in to our NeRAIN Volunteers.  They will analyze the data and then post it under the Reports section of the web page. 

If you find that you have entered some data incorrectly any time before you submit the form, you can either go back and change it, or click the button labeled "Reset".  The Reset button will completely clear your ENTIRE form and it will go back to the way it was before you entered anything.
